Existing PVG Scheme member application form

The following page is for preferred candidates who are being recruited into a job with Aberdeenshire Council which requires a PVG check and have already joined the PVG scheme. 

Please complete the following in your existing PVG scheme application form; 

Part A 

Complete A4, (if you select ‘yes’, please complete your email address in A5 / A6)

Part B

Completed all questions in Part B, fields highlighted in yellow on the left hand side are mandatory and must be completed.

Complete B1. If you do not know your PVG membership number (16 digits) please call Disclosure Scotland in Glasgow on 03000 2000 40

Enter any additional information where this is requested. (If you select ‘yes’ please provide details) e.g. B7 changes to personal details, please provide more details on a separate piece of paper

Part C

C1 / C2 Please read declaration, signing within the white box.

Before sending your application in the post back to Woodhill House. 

Please check you have completed all sections required; A4, Part B, signed and date Part C.

If you are unable to complete and post your application back to Woodhill House within 5 days, please contact your allocated HR Assistant. You can do this by replying to the email you received from them following your phone call to confirm the pre-employment checks required.

Please be aware any delay we experience in receiving your completed PVG application form will impact on the time your pre employment checks take to complete and be deemed satisfactory.
